When you come to your appointment please bring:
- Any relevant letters or reports from your doctor or hospital
- Any X-Rays, CT or MRI films and reports
- A list of all medicines you are taking
- Your ACC number, if you have one
- Your insurance details if applicable
If you need to cancel your appointment please telephone the office during business hours.
